Anybody have a Rent To Own Agreement?

Does anybody have a rent-to-own agreement handy I can use? I think we've got an RTO tenant for one of the flips, so I need to get something on paper for her to sign.

Looking to make sure she handles all of the repairs/maint for 2 years, and at the end of the 2 year period, she purchases the property for X price, and X amount of the rent will be applied as her down payment, etc.
